Architect, Engineer, Geologist and Landscape Architect Applicants
- To complete the application, please:
- Complete all questions, sign and date.
- Include a signed check or money order in the amount of $325.00 for the application & initial registration fee.
- Include completed, signed and dated “ Arizona Statement of Citizenship” form and supporting evidence
- Military status form
- Request Official license verification be sent to the Board
- Include a copy of the state’s statutes/rules that governed the license being granted
- Any applicable disciplinary or criminal documents
- Proof of Arizona residency - submit at least one (1) of the following:
- A valid Arizona driver’s license / ID Card -or- a current Arizona motor vehicle registration
- Documentation demonstrating a permanent address in Arizona on pertinent records such as: utility bill, mortgage payment, lease payment
- Military Form 2058
- Arizona Voter Registration

- Include documentation of passed AZ State Specific Land Surveying Exam
- Request Official license verification be sent to the Board
- Include a copy of the state’s statutes/rules that governed the license being granted
- Any applicable disciplinary or criminal documents
