Request to Send a Verification of Registration or Examination to Another Regulatory Board

The purpose of requesting a verification of registration or examination is to facilitate licensure in another jurisdiction or to update national council records for an individual licensed or designated in-training in Arizona.

Only an Arizona registrant or individual designated in-training may request a verification of their registration or examination. If you are not registered or designated in-training with the Board, you cannot request a verification.

The Board will only send a verification to another regulatory Board or National Council. Verifications cannot be sent to the requestor or other 3rd parties. 

If you wish to have the Board verify your registration/examination information and have it sent to another regulatory Board or National Council, please follow the instructions below. It may take up to thirty (30) days to process a verification request.

You will complete the webform, upload your completed verification form (complete sections I and II only please) and then make your payment. 

Each registration verification costs $25.00, regardless of which verification form is used (Arizona's or another state's). Verifications will be sent directly to the receiving State Board or National Council. 

If you intend to send another state's verification form to the Board and wish to pay online, please submit a scanned pdf of the verification form via the upload Verification Box on the Verifications webform on the Board's payment portal (link above).
