a) How do I complete the Certificate of Experience (COE) form?
You are responsible for filling out Page I, Section A of the form detailing your work experience and mailing three COEs to your present and past supervisors. The supervisor must fill out Section B. Two of the three COE forms must be filled out by supervisors who are professional registrants registered in the category for which registration is sought per A.A.C. R4-30-201(12-14). In the section marked "Time Worked (in months)", the sum of the time split between the four categories must total the amount of months of employment for the particular company. Please note that you can only receive 12 months credit for 12 months worked and a maximum six months of sub-professional credit. You will not receive double credit for school and work. The COE must be submitted to the Board directly by the supervisors.
b) Can I use a reference from a foreign country? An unregistered reference? Does the COE have to have a seal?
Supervisors filling out the form are not required to stamp the COE with a seal. They can either seal the document or enter a registration number if the supervisor does not have a seal. They must complete Page 2 in its entirety. Any references that are registered in a foreign country must provide their personal resume. Unregistered references must provide their resume as well as their evaluation. All COEs must be submitted by the supervisor/reference directly to the Board.
