Please make sure to check the website for the newest forms when it is time to apply for or renew registration. Do not save forms for reuse. If an application or renewal is submitted using an older version of a form it will be returned with a letter stating why the form was returned.
If your firm conducts fingerprinting in-house, please make sure you are following CJIS/CHRI procedures, specifically the following:
- The technician must complete [the AZBTR’s] Fingerprint Verification Form and seal the form and the completed fingerprint card in the tamper-evident envelope [underline added for emphasis]. The technician must then seal the envelope and write their name or identification across the edge of the seal. They are not to give the fingerprint card to the applicant without first sealing it inside the envelope.
- The applicant must provide the unopened envelope containing their fingerprint card and the Fingerprint Verification Form to the Board with their initial application package, certificate renewal package or annual fingerprint resubmission package.
Any submitted sealed Fingerprint Verification Forms received that are, or appear to be, tampered with or prepared in a manner that does not prevent tampering will be returned with a letter stating why the forms were returned.
