Alarm Industry Applicants
A Revised 'Alarm Agent Application Packet' and 'Controlling Person Application Packet' will become available Monday, March 7, 2022. The Board will not accept an older version of the 'Alarm Agent Application Packet' and 'Controlling Person Application Packet' after March 20, 2022.

**Please note** According to ARS 32-122.05(A) - An alarm business may not operate until the alarm business and each of its controlling persons submit applications and receive certification from the board.

Alarm Agent Application Packet
To complete the application, please:
- Answer all questions, sign and date.
- Have the Controlling Person of current alarm business employer sign.
- Include one current 2-inch by 2-inch passport quality photograph (No hats or head coverings (unless worn daily for religious purposes). No sunglasses. Clear lens glasses are acceptable if eyes are visible and there is no glare.)
- Include a copy of the applicant's fingerprint clearance card (front and back) OR section five completed by controlling person including the criminal background check report and documentation that applicant has applied for a clearance card. You can apply for the card on the Department of Public Safety Website.
- Complete, sign and date “Arizona Statement of Citizenship and Alien Status for State Public Benefits,” which is found within the Alarm Agent Application Packet, along with a copy of supporting evidence as listed on pages 3-5 of the document.
- Copy of a government issued photographic identification, pursuant to A.R.S. § 41-1080. (See the included document within the Alarm Agent Application Packet, which lists acceptable forms of ID headed by "Attention" and dated 8/9/2017).
- Sign and date Instructions for Alarm Agent Certification.

- Include a copy of the corporation, partnership, or limited liability company formation documents.
- Include a completed list (see packet) of names, residence addresses and date of births of each controlling person

Clearance card denied?
How to apply for a good cause exemption. If your clearance card application was denied because of a past criminal conviction, you can apply to the Arizona Board of Fingerprinting for a good cause exemption. You can learn more about the good cause exemption application at https://fingerprint.az.gov. The Board cannot issue or renew a certification for alarm agents or controlling persons without a clearance card. Please take steps now to determine if your clearance card application may be denied and apply for the good cause exemption well in advance of your renewal or application date.
Will my criminal history prevent me from obtaining a license?
An individual can request that the Board review their criminal history prior to submitting an application or submitting any fees.

Waiver of Initial Registration Fee
Per A.R.S. § 41-1080.01 "an agency shall waive any fee charged for an initial license for any individual applicant whose family income does not exceed two hundred percent of the federal poverty guidelines if the individual is applying for that specific license in this state for the first time."

Applicant Review and Challenge of Criminal History
If an applicant feels his/her criminal history record is inaccurate or incomplete, please refer to the appropriate contact below to begin the review and challenge process.
For a copy of an Arizona criminal history record:
The individual can contact Department of Public Safety Criminal History Records Unit at (602) 223-2222 to obtain a fingerprint card and a Review and Challenge Packet, or the individual can download information from the Criminal History Records Unit section of the DPS website. The Department of Public Safety provides the review and challenge packet pursuant to R13-1-08 of the Arizona Administrative Code.
This will check the Arizona criminal history only.
For a copy of an FBI criminal history record:
U.S. Department of Justice Order rules and federal law allow the subject of an FBI record to request a copy of his/her own record. The individual may submit fingerprints, an Applicant Information Form, and payment directly to the FBI according to the procedures in Title 28 Code of Federal Regulations 16.30 - 16.34.

An applicant, controlling person, alarm agent or home inspector will have no more than the 60-day substantive review period to challenge a criminal background check report.
A.R.S. 41-1093.05
Pursuant to section 41-1093.01, Arizona Revised Statutes, an agency shall limit all occupational regulation to regulations that are demonstrated to be necessary to specifically fulfill a public health, safety or welfare concern. Pursuant to section s 41-1093.02 and 41-1093.03, Arizona Revised Statutes, you have the right to petition this agency to repeal or modify the occupational regulation or bring an action in court of general jurisdiction to challenge the occupational regulation and to ensure compliance with section 41-1093.01, Arizona Revised Statues.
A.R.S.32-4303. Military education, training and experience
Notwithstanding any other law, the education, training or experience requirements for a license, certificate or registration issued pursuant to this title are completely or partially satisfied, as determined by the regulating entity, on presentation of satisfactory evidence that the applicant received substantially equivalent education, training or experience as a member of the United States armed forces or any national guard or other reserve component. The regulating entity shall work in conjunction with the department of veterans' services to access information regarding the applicant's military education, training or experience.
